Elective Request Paperwork
All forms here are on the GME website (http://www.bethisraelgme.org) that is password protected. There are more descriptions and explanations on that website. If you need a reminder of the password, please ask the Chief's.
These forms need to be turned into Cynthia 30 days in advance of the first day of the elective.
