How to discharge a patient for follow up at Ryan Nena
1. Fill out the top of the Referral form completely. Especially important is the patient’s contact telephone number and the name of the resident discharging the patient.
2. Unless you are making an appointment with a specific Ryan Nena resident (see # 6.a. below), you should leave the “Ryan—NENA Provider” field blank. A blank field here is the trigger that ensures the patient will be scheduled with an available Beth Israel resident.
3. Have completed referral form faxed to Ryan Nena (fax number is on form). Only the front side of the form need be faxed.
4. Give a copy to the patient and clearly describe the details of the follow up in the D/C summary.
5. No form faxed = no appointment. The form must be faxed.
6. There is no need to call Ryan Nena to schedule an appointment unless you want to schedule it with a particular provider.
a. To schedule with a particular resident provider, Ryan Nena should be called to see when they are available.
b. You still need to fax the form if you call (see #5).
7. Two appointment times have been blocked each day for new referrals—10am and 2pm. If a patient cannot make either, a call to Ryan Nena will be necessary.
8. Ryan Nena will accept faxed appointment sheets for next-day follow up appointments (including on Saturday or Sunday for a Monday appointment). No need to make a phone call.
9. If Ryan Nena cannot keep the exact appointment, they will call the patient themselves and re-schedule for either another time that day or the day before or after.
