Instructions For Completing Evaluations and Duty Hours
Evaluations and (for house staff) duty hours are now logged on New Innovations.
To access New Innovations: click here.
On the log on screen you will be asked for three things:
Institution: CHP
Username: (your username)
Password: (your password)
Should there be an erroneous evaluation on your list or if you have forgotten your Username/Password please contact Joan Neufeld via email at JNeufeld@chpnet.org or 212-844-1825.
